Explanation of Insurance and Billing
 
  Payment:
Payment is due at the time services are rendered. We accept cash, credit cards, and checks. Per our contracts with the insurance companies, we must collect all co-pays prior to your office visit, or your visit will need to be rescheduled. Our office will bill your insurance company for your visit.
 
If you have questions about your bill, please ask to speak with a member of our billing team so that she may provide answers and a resolution to your concerns. If we are a contracted provider with your insurance company and are able to verify and confirm coverage, you will only be responsible for your co-pay or co-insurance and deductible at the time of your visit.
 
Please note: Verification of coverage is not a guarantee of coverage. You will be considered responsible for all visits, labs, and procedures not covered by your insurance. You are responsible for knowing your policy's terms!
 
Returned Check Policy:
Non-Sufficient Funds checks will require complete payment in cash or certified funds for the amount of the check PLUS a $25 fee for checks under $50 and $30 for checks over $50.
 
Deductibles:
If your deductible has not been met, you will need to pay all charges until the deductible is met. If there is an overlap in payments, we will issue you a refund, or it will be applied to future visits.
 
Percentages due (co-insurance):
If your insurance policy only pays a percentage of your visit or procedure, an estimate of your amount owed will need to be paid the day of your visit or procedure. The percentage is based upon the allowed amount. If there is an overpayment, we will refund the difference to you or it will be applied to future visits.
 
Billing Invoices:
If you receive a bill from Women's Wellness Center because your insurance company has not issued payment to our practice, we ask that you contact your insurance company to help get the claim paid; otherwise the balance will become your responsibility. A finance charge will apply to bills that are not paid after the initial statement.
 
No Insurance Coverage:
For visits with a physician or nurse practitioner we offer a 30% prompt-pay discount for patients with no insurance coverage, or with whose insurance we are not participating. To receive this discount, payment is due at the time of service. This discount is not available for massage, health coach, or acupuncture services. Mental health counseling services vary in prompt-pay discount amount (depending on provider).
